*Please note your practice may need to upgrade the version of widget they are currently using to access the pre-appointment emails and pre-appointment questions function. Please ask your Practice Relationship Manager to check if your practice needs to update.
We have introduced a new questionnaire/eligibility feature that can apply to new or existing appointment types. Your practice relationship manager is available to assist you in setting these up however here is a step by step guide for your reference.
Navigate to Appointment types and either Crete a New appointment or edit an existing one. To Navigate form your main Appointuit practice portal:
- Select the Settings drop down box
- Select your branch name
- Select 'Appointment Types' from the list on the left
Set the rest of your Appointment parameters and Click the 'Add Questions' button to set-up questions that can force stop a patient booking depending upon their answers.
1) Set your question number/order
2) Type in your question
3) Select how you will allow your patients to select their answer
- Text - a patient will be able to freely write in their answer via a text box
- Checkbox - a patient can select multiple options from your pre-determined list
- Radio - a patient can select only one option from you pre-determined list
- Drop-down - a patient can select one of your pre-determined answers from a drop down box
4) Tick the box if you require the question to be answered before a patient moves on in the booking process.

When you have completed setting up your appointment press 'Save'
We have created some suggested COVID Vaccination Initial Appointment questions to assist with phase 1B of the roll out. These will need to be reviewed and amended as the government roll-out continues and eligibility criteria continues to evolve. Please feel free to use these as is or edit them as you see fit.
- Objective: Establish Eligibility
Question) Which criteria applies to you?
Answer type: Checkbox
Answers:
- I am aged 70 or over
- I am an aboriginal or Torres Strait Islander aged 65 or over
- I am a healthcare worker
- I have an underlying medical condition
- I am a critical and high risk worker including; defence force, police, fire emergency services and meat processing
- None of the above
Actions: If a patient selects anything BUT None of the above the booking process will continue. If they select ‘None of the Above’ It will force stop their booking and display your ‘halt’ message.
- Objective: Confirm they will provide proof of Eligibility
Question) I agree to bring in all supporting documentation to verify my eligibility. This includes an ID card or letter from my employer if I am eligible through employment OR a referral letter from my GP if I have an underlying medical condition.
Answer type: Radio
Answers:
- Yes
- No
Actions: If a patient selects ‘Yes’ the booking process will continue. If they select ‘No’ it will force stop their booking and display your ‘halt’ message.
- Objective: Gain consent
Question) I have read and understood the COVID vaccination consent policy. I understand I can discuss any concerns with the doctor or nurse and revoke my consent prior to or on the day of the vaccination.
Answer type: Radio
Answers:
- Yes
- No
Actions: If a patient selects ‘Yes’ the booking process will continue. If they select ‘No’ it will force stop their booking and display your ‘halt’ message.
